Documents necessary for issuing a diploma:

 

1. Summer practice diary.

 

2. Complex patient notebook.

 

3. A note from the library for not owing any books.

 

4. A note from the students’ dormitory (even if it has been used for 1 day in the first year).

 

5. Photo (passport format) - hard copy and an electronic file /PDF/ sent beforehand to e-mail: students.office@fdm.mu-sofia.bg

The photo file must be named with the student's faculty number, name and surname

 (Example: Е 2003 - Ivan Petrov)
